The World Health Organization has issued its first recommendation supporting the use of doxycycline post-exposure prophylaxis (doxy PEP) as a strategy to help prevent certain sexually transmitted infections (STIs). The guidance focuses primarily on populations at higher risk, particularly gay and bisexual men and transgender women who have had a recent bacterial STI. Doxy PEP involves taking a single dose of the antibiotic doxycycline shortly after unprotected sex, with evidence showing it can significantly reduce rates of syphilis and chlamydia, and to a lesser extent gonorrhea. The WHO emphasizes that the intervention should be implemented alongside, not in place of, existing prevention measures such as condom use, regular testing, and broader sexual health services. The recommendation reflects growing clinical trial data supporting the approach, while also noting concerns about antibiotic resistance that require ongoing monitoring. While we are happy to see sexual health becoming normalized, particularly for queer populations, popping a pill for everything from HIV prevention to hair thinning, premature ejaculation, and erectile dysfunction does raise some health concerns.
